Misprogramming patient-controlled analgesia levels causes dosing errors.

چکیده
74 P&T® • February 2012 • Vol. 37 No. 2 Errors in initiating PCA infusions can occur at any point in the programming process. By using a limited number of standard concentrations, standardizing order forms, implementing smart PCA pumps with dose error-reduction software (DERS), and requiring an independent double-check of the PCA programming, hospitals can develop effective tools to help reduce PCA dosing errors. However, the risk of a PCA error increases significantly when there are atypical orders, nonstandard concentrations, and unusually high doses. In reports received by the ISMP related to concentrations of PCA infusions, several common findings merit attention:

Comparison of the Efficacy and Safety of a Pharmacokinetic Model-Based Dosing Scheme Versus a Conventional Fentanyl Dosing Regimen For Patient-Controlled Analgesia Immediately Following Robot-Assisted Laparoscopic Prostatectomy
Conventional, intravenous, patient-controlled analgesia, which is only administered by demand bolus without basal continuous infusion, is closely associated with inappropriate analgesia. Pharmacokinetic model-based dosing schemes can quantitatively describe the time course of drug effects and achieve optimal drug therapy. We compared the efficacy and safety of a conventional dosing regimen for ...
متن کاملEvaluation of Use of Electronic Patient Controlled Analgesia Pumps to Improve Patient Safety in an Academic Medical Center
Patient controlled analgesia (PCA) and Patient-controlled epidural analgesia (PCEA) pumps are methods of pain control with complex smart infusion devices and are widely used in hospitals. Smart PCA/PCEA pumps can be programmed with the dose and rate of medications within pre-set ranges. However, adverse effects have been reported associated with these pumps' use. In this paper, we describe a pr...
متن کاملPatient-controlled epidural analgesia is superior to nitrous oxide inhalation in controlling childbirth pain
Objectives: To compare the effects of different labor analgesia in controlling childbirth pain and seek more effective and reliable pain management strategy during natural childbirth in order to reduce maternal pain and promote natural childbirth. Methods: 400 cases of successful delivery with labor analgesia in our hospital during 2010 and 2014 were enrolled in this study, including 200 cases ...
متن کاملAcute postoperative pain management: focus on iontophoretic transdermal fentanyl
Despite progress in the management of chronic pain, acute pain remains an issue for many postoperative patients. Although patient-controlled analgesia (PCA) has demonstrated efficacy and patient satisfaction, current techniques using intravenous (IV) administration present limitations, including the risk of programming errors and the potential to limit patient mobility due to pumps, lines, and ...
متن کاملA Randomized Controlled Trial Comparing Preemptive Versus On Demand Administration of Analgesics after Mohs Surgery and Cutaneous Reconstruction
BACKGROUND: Mohs Micrographic Surgery (MMS) affords a high cure rate for cutaneous carcinoma. OBJECTIVE: This study was designed to characterize the pain patients experience after MMS and reconstruction of the head and neck, and to determine if preemptive dosing of analgesics was superior to patient‐initiated dosing on an "as needed" basis. METHODS: A controlled trial of 200 subjects undergoing...
